Hospice At Home
This service runs 7 days per week 24 hours a day offering support to patients and their families in collaboration with other Health Care Professionals.
Referral is via the patients District Nurse and the Hospice@Home team consists of a Clinical Nurse Specialist, Registered Nurses, Experienced Health Care Assistants, a Social Worker, and Aromatherapy, Reflexology and Physiotherapy Assistants.
The team focuses on maximum choice for palliative care patients and appreciates the value of them being able to be cared for in their own home in the last weeks or days of life. Additionally respite relief for carers can be offered.
We make every effort to provide a nurse for anyone within the Eastbourne, Polegate, Seaford, Pevensey, Hailsham, Heathfield, Horam and Uckfield areas.
The service which initially developed in conjunction with the local community Health Care NHS Trust continues to be managed and funded by St Wilfrid's Hospice.
