In-Patient Unit
Assessment
The admission assessment is carried out by one of our doctors accompanied by a nurse and is the process we use for gaining information about the patient, their current health status and their needs together with those of your main carer or family.
We take a holistic approach, meaning that we get to know our patients as a whole person rather than concentrating solely on their illness. A Doctor assesses the person each day together with ongoing assessments by the nurse looking after them.
Named Nurse
Each one of our patients has a named nurse allocated to them, who in most cases will be the nurse present during their admission assessment. Whilst many members of the Hospice Team will care for them during their stay, the named nurse will plan and co-ordinate their care, working in partnership with them, their family and friends. Named nurses endeavour to care for their allocated patients when they are on duty, and will help with their future care needs and liaise directly with Macmillan and District nurses.
Discharge planning
The Hospice is a short stay unit, and most distressing symptoms can be well controlled in about two weeks.
